Which sensor is being replaced?
Twelve digits. Take it from the app on their phone before a new session is started.
Carry on without it. The claim is still settled here.

Where to find the serial number

  • In the app, first — Connections, then Sensor, listed as SN. Check it before a new session is started, because the number can drop out of the app once the failed session ends.
  • On a receiver — Menu, then Information, then Sensor, then Session info.
  • On the box or applicator — the twelve digits printed after 21, for anyone who has kept them. Most people have not.
  • Not on the sensor itself. The worn sensor carries no readable number, so if the app has cleared it and the packaging is gone, tick the box above and carry on. It does not hold up the replacement.
Type of diabetes
Recorded so Dexcom can see which groups are affected. It does not change eligibility.

What happened to the sensor

Two details and two checks. Every replacement is settled here, so the checks decide how the claim is recorded, not whether it goes ahead.

The day the session was started, not the day the box was opened.

Two checks for the record

1
Was the sensor still inside its wear period when the problem started?
2
Has a replacement already been issued for this sensor?

Good to know

  • A sensor that stops inside its wear period is recorded as a product failure, with no limit on how many times it can be replaced.
  • Anything outside that is recorded as a courtesy replacement. It is still settled here.
  • Either way the person leaves with a sensor. Nobody is sent away to make a call.

Answer both checks to continue

Both answers lead to a replacement. They set how the claim is recorded for Dexcom.

Who is the replacement for?

Helping people with type 2 diabetes start on Dexcom ONE+

This service hands a free ONE+ starter pack to people living with type 2 diabetes, so they can see what their glucose is doing rather than guess at it. Ask the person for the code they were sent by email for the starter pack, and enter it below.

Redeem their code

Enter the code exactly as it appears in their email.

Demo only, a valid code is ONE 3339
Code valid, one Dexcom ONE+ sensor
Does this person have diabetes?
The starter pack is for people with type 2 diabetes. Dexcom ONE+ itself is for people with diabetes mellitus aged 2 and over, where checking glucose is already part of their care.

What they are redeeming

  • One Dexcom ONE+ sensor, at no cost to them, handed over here in store.
  • Ten days of wear, with a twelve hour grace period after that.
  • Readings on their phone, day and night, with high and low alerts they can set.
  • Waterproof, so showering, swimming and exercise all carry on as normal.

Before you start

  • Check the code is on their own phone, not forwarded from someone else.
  • They will need a compatible smartphone to see readings.
  • Set aside about ten minutes. The sensor goes on before they leave.

Who is it for?

Taken so the sensor is registered to them and the code cannot be used twice.

A one time code is sent to this number to verify it. This must be the number the offer code was sent to.

Why the number is verified

  • It confirms the code belongs to the person standing in front of you.
  • It closes the code, so it cannot be redeemed again at another store.
  • It gives Dexcom a clean record of who the sensor went to.
Verified — hand over one Dexcom ONE+ sensor and set it up together

Hand it over, and show them how

Work down the list with them. The sensor goes on here, so they leave with readings running.

Worth saying out loud

  • It is waterproof. Showering, swimming and exercise are all fine.
  • Readings show the direction glucose is heading, not just the number.
  • Checking, not testing. There are no good or bad numbers, only time in range.
  • Any questions after today, they can come back to the pharmacy first.
All six are ticked before the handover can be recorded. 0 of 6 done.

Before they leave the counter

  • Check the app is showing a warm up countdown, so you know the pairing worked.
  • Tell them the sensor runs to day ten, and to set the change reminder now.
  • Invite them back around day eight to talk about what they saw and what comes next.
  • Your campaign stock is topped up automatically against this redemption.
